Treatment Agreement
for Adults – Mental

Boncura GGz provides the necessary treatment or guidance as described in the treatment plan, in accordance
with the applicable indication and the General Terms and Conditions for Mental Health Care (GGz). These terms
are available at https://boncura.nl/algemene-voorwaarden. Treatment plans may be adjusted in consultation
between the client and the lead practitioner. If the treatment proves ineffective, it may be terminated, after
which alternative treatment within Boncura or a referral to another healthcare provider may be considered and
indicated. - Start and End of the Agreement
This agreement begins on the date of signing and ends on a date to be determined. Boncura organizes treatment as briefly as possible and as long as necessary, within the requirements of the health insurer as contractually agreed with Boncura (see also https://boncura.nl/kosten). Boncura will inform the client promptly
of any changes. The agreement may be terminated early in accordance with the General Terms and Conditions
for Mental Health Care (GGz). Upon termination, ongoing appointments or outstanding payments will be settled
according to the applicable conditions. - Treatment and Guidance
The treatment will be outlined in a care or treatment plan. It is important that all agreements are adhered to in
order to ensure the success of the treatment. Failure to cancel appointments in time (at least 24 hours in
advance) will result in a ‘no-show’ fee, in accordance with Article [specific article] of the General Terms and
Conditions for Mental Health Care (GGz) and the conditions at https://boncura.nl/no-show.
Treatment and guidance may be offered individually or in groups, depending on what is appropriate within the
treatment plan. More information about group treatment is available at https://boncura.nl/kosten.
Referral letters will be screened based on inclusion and exclusion criteria. If it becomes evident during the
intake process that the client’s needs do not align with Boncura’s services, Boncura will provide an alternative
recommendation. Intake costs will be declared to the health insurer and will count towards the client’s
deductible. - Billing and Additional Conditions
For health insurers with which Boncura has a contract, care will be billed directly to the insurer. The deductible
will be charged to the client by the insurer. Clients with international insurance must verify whether the
treatment is covered; if not, the client is responsible for paying the costs monthly. Boncura accepts no liability
for costs not covered by a foreign insurer.
Additional Conditions for ONVZ
For clients with ONVZ insurance, additional conditions apply. By signing, the client agrees to these terms, as
described at https://boncura.nl/kosten, including any additional costs. - Consent for Data Processing
Boncura GGz processes the client’s personal data in accordance with the General Data Protection Regulation
(GDPR). This data is used for the execution of the treatment and may be shared with involved healthcare
providers and insurers. For more information, please refer to the privacy statement at
https://boncura.nl/privacyverklaring. By signing, the client acknowledges being informed of this.
